The New York Department of Health (DOH) is conducting a survey of health care providers’ use of telehealth services. DOH’s goal in conducting the survey is to gain knowledge of providers’ experiences using telehealth during the COVID-19 Public Health Emergency, as well as their continued use of Telehealth.

The survey will close on October 2, 2023. DOH indicates the survey should take approximately five minutes to complete.
