The New York Department of Health recently posted links to a podcast addressing Transforming Care for Multi-Visit Patients. This highlights lessons learned from the NYS DOH 2022 Medicaid Accelerated eXchange series program teams.
The Accelerated eXchange series program was a Delivery System Reform Incentive Program initiative from DSRIP’s fourth round. The podcast includes presentations from Dr. Amy Boutwell (developer of the Multi-Visit Patients method), who also hosts conversations with Accelerated eXchange action team members from eight participating hospitals and with Dr. Douglas Fish (Chief Medical Officer of DOH’s Medicaid Program). They address their experience with the initiative and the impact the efforts have made on the lives of patients and families.
